心房颤动脉冲电场消融的再认识Re-understanding of pulsed electric field ablation for atrial fibrillation

摘要(Abstract):
经导管脉冲电场消融(PFA)治疗心房颤动在全球得到广泛的临床应用,积累了大量有效性和安全性的临床证据,近年来也观察到一些与该技术相关的不良事件,如消融相关的冠状动脉痉挛、溶血及术后急性肾功能损伤、术后无症状或症状性脑血管事件等。本文结合近两年PFA的最新基础和临床研究进展,从脉冲电场的场强阈值、PFA的热效应、接触力和消融次数等生物物理学效应方面对PFA是否具有组织选择性,溶血和微泡的发生机制,损伤的形态和深度等进行阐述,以期帮助临床医师更全面地了解和应用PFA技术。
