体外膜肺氧合患者早期心脏康复的临床获益Clinical benefits of early cardiac rehabilitation in patients after extracorporeal membrane oxygenation

摘要(Abstract):
目的 回顾性分析体外膜肺氧合(ECMO)术后早期心脏康复的临床获益。方法连续入选2020年3月至2023年1月在兰州大学第一医院心脏中心接受ECMO支持治疗且分别于撤机前后进行心脏康复干预的重症患者76例。剔除中断治疗的4例,最终纳入72例。心脏康复干预方案为:渐进性康复训练方案,即由被动到主动,活动强度由弱到强,时间由短到长。根据心脏康复开始时机分为撤机前组(30例)和撤机后组(42例)。结果 两组患者临床基线资料比较,差异均无统计学意义(均P>0.05)。72例患者均未发生心脏康复相关的不良事件。撤机前组的住院天数(12.60±3.68)d比撤机后组(17.88±6.76)d缩短,差异有统计学意义(P=0.001);出院前离床率撤机前组(73.3%)高于撤机后组(45.2%),差异有统计学意义(P=0.029);两组间的转归比较差异无统计学意义(P=0.111)。结论 ECMO患者的早期心脏康复不仅安全可行,而且有助于缩短住院时间、提高出院时的离床率。
