CT血管造影结合血管内超声引导无对比剂完成左回旋支重度狭窄介入治疗1例Coronary computed tomographic angiography and intravascular ultrasound assisted no-contrast left circumflex percutaneous coronary intervention:a case report

摘要(Abstract):
碘对比剂过敏患者经皮冠状动脉介入治疗(PCI)是冠心病介入治疗的难点之一。如何安全、高效、无对比剂或极低剂量对比剂完成冠状动脉病变介入治疗是对术者的挑战。本文报道1例对碘对比剂过敏患者采用CT血管造影(CTA)结合血管内超声(IVUS)引导无对比剂PCI。术前冠状动脉CTA和X线采集的患者气管、脊柱解剖位置信息与冠状动脉造影信息匹配、融合,在CTA图像实时指导下,指导指引导管、导丝的放置,并从不同角度判断导丝在管腔内的位置。采用IVUS明确血管真腔、病变位置、病变长度、参考管腔直径及落脚点,成功行左回旋支支架置入术。以此探讨CTA融合IVUS引导无对比剂介入治疗技术,分析其适应证、术前准备、术中操作技巧、并发症预防及处理等,为临床处理此类病变提供一定的参考。
