摘要(Abstract):
经导管主动脉瓣置换术(TAVR)在我国临床应用已逾15年,随着首批置入瓣膜进入耐久性衰减期,再次TAVR(Redo-TAVR)成为我国结构性心脏病领域亟待规范的重要临床场景。我国接受首次TAVR患者具有二叶式主动脉瓣比例高且瓣膜钙化重,风湿性病因和主动脉瓣反流常见等独特流行病学特征,且首次TAVR以自膨胀式瓣膜为主(占比93%),与欧美人群存在显著差异。本共识基于国际前沿研究数据及我国临床实践,系统阐述Redo-TAVR的适应证、禁忌证、术前评估、手术策略、临床效果、并发症防治、围手术期管理及远期随访,旨在规范我国Redo-TAVR临床操作,降低手术风险,改善患者长期预后,推动我国瓣膜性心脏病全生命周期管理体系建设。
