摘要(Abstract)：

高龄妊娠合并急性ST段抬高型心肌梗死的患者，国内外较为罕见，目前尚无相关指南指导临床治疗。本病例报道1例39岁高龄孕妇突发胸痛入院，心电图示广泛前壁ST段抬高型心肌梗死。冠状动脉造影示：左前降支近段次全闭塞。给予再灌注及血运重建治疗，恢复心肌梗死溶栓治疗试验血流分级Ⅲ级。完善血管内超声检查，明确左前降支自发血肿导致闭塞的病因。术后根据冠状动脉病变情况选择合理的抗血小板、抗凝治疗方案，并选择恰当的时机终止妊娠，成功分娩出男活婴1名。1、3、6个月随访母婴健康。本病例报道通过对救治过程的体会及总结，结合文献，探讨该类疾病的发病机制，对血运重建的策略、抗栓药物的选择及终止妊娠的时机等方面进行深入讨论，以供临床借鉴。
