单导管隔离肺静脉前庭的方法学评价Feasibility and efficacy of single catheter technique for pulmonary vein antrum isolation

摘要(Abstract):
目的探讨单导管技术隔离肺静脉前庭(PVA)的可行性和有效性。方法对55例抗心律失常药物治疗无效的阵发性心房颤动(房颤)患者进行初始环肺静脉消融后,应用单导管技术标测和补充消融缝隙。结果初始环肺静脉消融后PVA的电学隔离率:右侧PVA为61.8%(34/55),左侧PVA为27.3%(15/55),双侧PVA为18.2%(10/55)。沿右侧PVA消融线有25个缝隙(17例患者有单个缝隙,4例患者有2个缝隙),沿左侧PVA消融线有49个缝隙(32例患者有单个缝隙,7例患者有2个缝隙,1例患者有3个缝隙)。经补充消融后,消除了所有缝隙传导。操作时间平均为154±29(99~204)min,X线曝光时间平均为32±7(19~49)min,放电时间53±10(31~72)min。结论阵发性房颤消融时,应用单导管技术定位缝隙实现PVA的完全隔离具有可行性和有效性。
