经冠状动脉灌注消蚀治疗心律失常

摘要(Abstract):
<正> 心律失常病源灶、折返传导径路及房室结等都有一定的冠状动脉小分支供血,精细的导管技术能选择性地进入这些小分支并实施血管暂时性嵌顿和灌注冰盐水、药物(如普鲁卡因酞胺、利多卡因、乙酰胆碱),或化学物质(如酒精、苯酚等),阻断病灶心肌细胞的供血或直接消蚀破坏,从而永久性地终止心律失常.此为某些顽固性心律失常病者提供又一有效的选择手段.自80年代中期,Chilson、Dami-ano及Inoue等学者分别进行了冠状动脉灌注消蚀的动物实验研究.数年后Brugada等将此技术引入临床,成功地获得对室性、房性心动过速的控制及房室旁道、房室结的消蚀.国内华伟等进行了实验性室性心动过速(简称室速)的冠状动脉灌注消蚀研究,但尚未见临床应用的报道.
