Lawnest血管内抓捕器用于慢性完全闭塞病变的逆向介入治疗1例Using of Lawnest,an intravascular snare in retrograde percutaneous coronary intervention for chronic total occlusion:a case report

摘要(Abstract):
冠状动脉慢性完全闭塞(CTO)病变仍是经皮冠状动脉介入治疗(PCI)领域面临的难题。逆向导丝技术是现代CTO-PCI的三大策略之一,主要步骤包括逆向导丝通过侧支通道、逆向导丝通过闭塞段以及正向导丝轨道的建立。本病例报道1例左回旋支(LCX)开口闭塞且被左主干-左前降支(LM-LAD)原支架所遮挡,逆向导丝通过LCX闭塞后只能向LAD远段方向走行,传统方法难以建立正向轨道的病例。采用国产Lawnest血管内抓捕器,在LAD血管内抓捕逆向导丝,并将逆向器械拉到正向指引导管内,从而建立正向轨道手术完成。冠状动脉内抓捕器为逆向CTO-PCI提供了新的解决思路,适用于逆向导丝难以进入正向导管的CTO病变患者。
