冠状动脉支架膨胀不全处理策略的最新进展Advances in treatment strategies for coronary stent underexpansion

摘要(Abstract):
支架膨胀不全作为经皮冠状动脉介入治疗中常见且棘手的并发症,尤其多发于严重钙化、分叉及慢性完全闭塞等复杂冠状动脉病变中,与支架内再狭窄及支架内血栓形成等严重不良心血管事件风险增加密切相关。腔内影像学技术的应用为精准识别支架膨胀不全、量化评估其严重程度及解析潜在机制提供了关键依据。本文系统回顾了支架膨胀不全的定义与诊断标准,重点综述腔内影像学在临床评估中的指导价值,总结当前主流处理策略的循证医学证据,涵盖经皮冠状动脉内冲击波球囊导管成形术、准分子激光冠状动脉斑块消蚀术、冠状动脉斑块旋磨术等特殊技术的应用进展,以期为支架膨胀不全的规范化处理提供参考。
