摘要(Abstract)：

目的系统评价导管消融治疗心房颤动合并心力衰竭患者的安全性和有效性。方法系统检索Embase、Pub Med、Cochrane图书馆、Clinical Trials.gov、中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、中国科技期刊数据库(VIP)以及万方数据库,筛选并纳入采用导管消融治疗心房颤动合并心力衰竭患者的试验,评价纳入研究的方法学质量,并应用STATA 12.0软件进行分析。结果共入选15项研究,累计2026例患者,其中686例接受导管消融治疗。Meta分析结果显示,导管消融术后左心室射血分数(left ventricular ejection fraction,LVEF)显著升高(WMD 12.48%,95%CI 9.49%~15.47%,P<0.001),但研究间存在显著异质性(I2=93.3%,P<0.001)。Meta回归显示,纳入人群的冠心病患者比例与消融术后LVEF升高存在负相关性(β=-0.86)。导管消融相关严重并发症的平均发生率为(4.8±3.6)%。结论导管消融能显著增加心房颤动合并心力衰竭患者LVEF值而有效改善心功能,且其并发症发生率与总体相当。
