摘要(Abstract)：

经皮冠状动脉介入治疗（PCI）过程中导丝断裂是少见但严重的并发症，其发生机制及应急处置目前尚无明确的指南推荐。导丝断裂的原因可归纳为解剖因素、操作因素及器械特性等；若处置不当，则可导致冠状动脉血栓、穿孔，甚至危及生命。本病例报道1例具有警示意义的特殊病例，PCI术中导丝自发断裂，导丝残段逆向移位，致冠状动脉及升主动脉穿孔，引发急性心脏压塞。经急诊心包穿刺引流及外科开胸手术后转归良好。本病例的处置经验为临床上带来一定借鉴价值。
