睡眠时长变短对经皮冠状动脉介入治疗后发生再次血运重建事件的影响The effect of shorter sleep duration on revascularization events in patients after percutaneous coronary intervention

摘要(Abstract):
目的 评估睡眠时长变短对行经皮冠状动脉介入治疗(PCI)冠心病患者术后发生再次血运重建事件的影响。方法 连续纳入中国医学科学院阜外医院冠心病中心16 948例接受PCI的注册登记患者,根据术后随访患者睡眠时长的变化分为睡眠时长较术前减少,即睡眠时长变短组(1 826例),其余患者为对照组(15 122例),主要研究终点为再次血运重建事件,通过多因素Cox回归探讨睡眠时长变短与再次血运重建事件的关系。结果 患者平均随访时间为(3.0±1.6)年,共1 563例(9.2%)患者发生再次血运重建,其中睡眠时长变短组216例(11.8%),对照组1 347例(8.9%),差异有统计学意义(P<0.001)。校正混杂因素后,与对照组相比,睡眠时长变短组患者发生再次血运重建事件的风险显著升高(HR 1.407,95%CI 1.217~1.627,P<0.001)。考虑死亡事件的竞争风险模型结果相似,显示睡眠时长变短组患者发生再次血运重建事件的风险更高(HR 1.404,95%CI 1.215~1.622,P<0.001)。对年龄、性别进行亚组分析,在高龄、低龄、男性、女性亚组人群中,与对照组相比,睡眠时长变短组患者发生再次血运重建事件的风险升高。结论 行PCI的冠心病患者术后睡眠时长变短与高再次血运重建发生风险有关。
