摘要(Abstract)：

重度主动脉瓣狭窄(AS)患者行经导管主动脉瓣置换术(TAVR),术前合并心房颤动(AF),或术后新发心房颤动(NOAF)在临床上比较常见。AF是TAVR术后死亡、卒中等不良心脑血管事件的重要影响因素。术前AF或术后NOAF患者TAVR术后抗栓治疗出血并发症发生率高,死亡率也明显增高,严重出血并发症风险给TAVR患者术后抗栓治疗带来了极大的挑战。对于不能长期耐受抗凝药物治疗且具有出血高风险的AF患者,左心耳封堵术(LAAC)是一种安全、有效预防卒中的方法。接受TAVR治疗的AF患者,可行LAAC以降低长期使用抗凝药物带来的出血风险。目前,TAVR术后择期行LAAC在国内外已较为普遍,但也有少数关于TAVR联合LAAC"一站式"手术可行的报道。本文重点就TAVR联合LAAC"一站式"手术的可行性和安全性进行综述。
