定量血流分数及模拟残余定量血流分数与血流储备分数在经皮冠状动脉介入治疗中的对比分析The clinical significance of quantitative flow ratio and residual quantitative flow ratio in percutaneous coronary intervention:a comparative analysis with fractional flow reserve

摘要(Abstract):
目的 探讨定量血流分数(QFR)及模拟残余定量血流分数(rQFR)与血流储备分数(FFR)指导的经皮冠状动脉介入治疗(PCI)策略的一致性。方法 本研究为单中心回顾性研究,纳入2016年3月1日至2024年3月31日北京大学第三医院接受PCI且术前及术后均测量FFR的临界病变患者。通过冠状动脉造影计算QFR和rQFR,并与PCI术前和术后的FFR进行比较。本研究的主要终点是12个月内主要不良心脏事件,包括全因死亡、非致死性心肌梗死和因靶血管缺血导致的再次血运重建。次要终点定义为12个月内心绞痛复发。结果 608例筛查出127例患者(共128支目标血管)最终纳入分析,其中108例患者完成了为期12个月随访。有2例患者发生主要终点事件,而有19例患者出现心绞痛复发次要终点事件。PCI术前的FFR和QFR分别为0.73(0.69,0.76)和0.72(0.68,0.76),差异无统计学意义(P=0.264);PCI术后的FFR和QFR分别为(0.86±0.05)和(0.87±0.05),差异无统计学意义(P=0.267)。然而,rQFR(0.89±0.05)分别与术后FFR、QFR比较,差异均有统计学意义(均P<0.001)。rQFR与PCI术后FFR相关系数为0.666,而PCI术后QFR与FFR相关系数为0.787。rQFR指导的支架长度30.00(21.30,40.00)mm和支架直径2.80(2.50,3.00)mm分别与实际术者选择的支架长度32.50(26.00,44.50)mm(P<0.05)和直径3.00(2.75,3.00)mm(P<0.001)相比,差异均有统计学意义。ΔQFR对12个月内心绞痛发作有一定程度的预测价值[曲线下面积(AUC)=0.713,95%CI 0.619~0.796,P=0.002],这与ΔFFR有相似之处(AUC=0.681,95%CI 0.585~0.767,P=0.013)。结论 rQFR、PCI后QFR以及PCI后FFR之间存在显著的相关性。rQFR模拟的支架尺寸比术者通过冠状动脉造影确定的尺寸更小。此外,PCI后FFR或QFR的显著提升可能与减少心绞痛复发相关。
