过氧亚硝酸阴离子加重急性心肌梗死患者缺血-再灌注心肌损伤Peroxynitrite aggravates myocardium injury in acute myocardial infarction during ischemia-reperfusion

摘要(Abstract):
目的研究急性心肌梗死患者缺血-再灌注过程中血浆过氧亚硝酸阴离子(ONOO-)与心肌损伤的关系。方法30名急性心肌梗死(均为前降支近中段单支病变)并急诊成功行支架干预术的患者为观察组,分别取不同时间点(24h、72h)的外周静脉血,据cTnI浓度分为轻度心肌损伤组(cTnI<30μg/L,16例)和重度心肌损伤组(cTnI≥30μg/L,14例);选择同期门诊体检的健康志愿者24名为对照组。用酶联免疫分析法检测血浆中硝基化酪氨酸(NT,ONOO-标志物)含量。结果急性心肌梗死患者轻度心肌损伤组和重度心肌损伤组PCI术后24h、72h血浆NT水平皆高于对照组(24h:1.08±0.35nmol/g蛋白、1.23±0.32nmol/g蛋白比0.51±0.12nmol/g蛋白,P<0.001;72h:1.05±0.21nmol/g蛋白、2.35±0.47nmol/g蛋白比0.51±0.12nmol/g蛋白,P<0.001);重度心肌损伤组术后72h血浆NT水平显著高于轻度心肌损伤组(P<0.001),而在术后24h两者之间差异无统计学意义(P>0.05)。结论急性心肌梗死患者缺血-再灌注后血浆ONOO-水平随心肌损伤程度的增加而增高,提示过氧亚硝基阴离子可能参与了急性心肌梗死患者缺血-再灌注心肌损伤。
