对比剂脑病合并药物诱导的极重度血小板减少症1例

摘要(Abstract)：
<正>1临床资料患者男,46岁。2014年12月17日因"胸痛伴有后背痛、出汗2.5 h"就诊于中国中医科学院望京医院。既往吸烟20余年,平均每日吸烟约30支。实验室检查肌钙蛋白升高,心电图未见ST段抬高(图1)。内科查体未见明显阳性体征。入院诊断"急性非ST段抬高型心肌梗死"。给予负荷剂量阿司匹林300mg+氯吡格雷300 mg后,行冠状动脉造影示:第一钝缘支散在斑块形成,近端完全闭塞病变,可见大量血栓影(图2),心肌梗死溶栓治疗试验(thrombolysis in myocardial infarction,TIMI)血流分级0级;左前降支(left anterior descending,LAD)中段局限性狭窄约90%,TIMI血流分级Ⅲ级(Medina 1.0.1)。
