冠状动脉腔内影像学指导左主干病变介入治疗

摘要(Abstract):
<正>左冠状动脉主干(简称左主干)是心肌供血的主要通道。既往研究指出,在诊断性冠状动脉造影患者中,左主干病变发生率为5%~8%,大多数位于左主干远端,且斑块常累及左前降支和(或)左回旋支开口[1]。传统认为,冠状动脉旁路移植术是严重左主干病变患者的优先疗法,但近年来多个随机试验(SYNTAX[2]、PRECOMBAT[3]、EXCEL[4]和NOBLE[5])提示,对低、中危的左主干远端复杂分叉病变,经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)可获得与冠状动脉旁路移植术相似的疗效。
