单电极与不同间距双电极射频消融——猪心室肌的对比实验研究

摘要(Abstract):
本研究用单电极和不同极间距的双电极(5mm,10mm)分别消融离体猪心室内膜,各种方法所用射频能量均分为8组(≤100焦耳,101~200焦耳,201~300焦耳,301~400焦耳,401~500焦耳,501~600焦耳,601~1000焦耳,≥1000焦耳),结果发现,双极消融所造成的组织损伤分布于两电极与组织的交界面处以及两电极之间.各相同能量组三种方法所消融心室肌肉膜深度无统计学差异,而在各能量组中5mm及10mm极间距双极消融面积均大于单极消融面积(P<0.05).在能量≤500焦耳时,5mm极间距双极消融面积大于10mm极间距双极消融面积(P<0.05).当能量大于500焦耳时.二者消融面积无统计学差异.提示双极消融与目前普遍采用的单极消融相比可明显扩大消融面积而不增加消融深度.推测双极消融法应用于临床可能比单极消融更有效而不增加心脏穿孔的危险性.目前国内、外常用的单次有效射频能量多不超过500焦耳,本实验提示,在小于500焦耳能量下,5mm间距双极消融效果优于10mm间距双极消融效果.
