微导管保护下双指引导管技术旋磨左主干真分叉病变1例One case of rotational atherectomy for true bifurcation lesion of the left main coronary artery using the dual-guide catheter technique under microcatheter protection

摘要(Abstract):
冠状动脉钙化病变在冠心病患者中十分常见,因其在介入治疗过程中并发症多,成为介入医师面临的一项重大挑战。钙化病变限制了支架或球囊的通过与扩张,导致支架贴壁不良、膨胀不全等围手术期并发症,远期可出现支架内血栓形成等不良事件。钙化病变的预处理方式逐渐发展,冠状动脉斑块旋磨术、冠状动脉轨道旋磨术、准分子激光斑块消蚀术及冲击波球囊导管成形术显著改善了冠状动脉钙化病变的治疗效果,增加了介入医师的信心。但所有手术方案均需要术者对病变进行充分的评估与准备。本病例报道1例微导管保护下双指引导管技术旋磨左主干的真分叉病变,为旋磨处理分叉钙化病变提供一种思路,以期尽量减少相关并发症,提高手术安全性,但涉及具体病变还需具体分析。
