体外膜肺氧合辅助循环崩溃高风险经导管主动脉瓣置换术技术要点专家共识Expert consensus on the technical key points of extracorporeal membrane oxygenation assisted hemodynamic collapse high-risk transcatheter aortic valve replacement

摘要(Abstract):
经导管主动脉瓣置换术(TAVR)患者围术期发生心搏骤停、急性心力衰竭、恶性心律失常、急性肾功能衰竭等严重并发症时,应用体外膜肺氧合(ECMO)等循环辅助支持可改善患者预后。但目前对于ECMO辅助循环崩溃高风险TAVR的技术要点尚未形成系统、规范共识。为此,由中国医疗保健国际交流促进会胸痛学分会、山东省医学会心肺复苏与体外生命支持多学科联合委员会牵头组织国内急危重症、心血管内外科、医学影像、康复医学等领域的专家,梳理了国内外最新循证医学证据和重要进展,结合专家临床实践经验,共同制订了《体外膜肺氧合辅助循环崩溃高风险经导管主动脉瓣置换术技术要点专家共识》,旨在规范TAVR围术期ECMO的主要技术要点,以期使ECMO更好服务于TAVR围术期管理,进一步提高TAVR患者的手术成功率,改善患者预后。
