摘要(Abstract)：

目的评估WATCHMAN左心耳封堵器经皮左心耳封堵术经验的累积对短期手术效果的影响。方法连续入选武汉亚洲心脏病医院2014年4月至2015年5月90例行经皮左心耳封堵术患者,按手术时间先后顺序分为3组:组1(2014年4月至2014年9月)第1~30例;组2(2014年10月至2014年12月)第31~60例;组3(2015年1月至2015年5月)第61~90例。分别对3组患者手术相关数据进行分析。结果患者平均年龄(65.7±7.6)岁,CHA2DS2-VASc评分为(3.8±1.5)分。手术成功率为94.4%。组1、组2、组3患者手术操作时间分别为65(53~90)min比48(42~63)min比46(41~56)min(P<0.001);X线透视时间分别为14(10~20)min比9(6~15)min比8(6~13)min(P<0.001);对比剂剂量分别为94(70~140)ml比72(50~100)ml比68(50~92)ml(P<0.001)。封堵器压缩比随学习曲线的延长逐渐增大,三组比较差异有统计学意义(15.0%比22.0%比25.0%,P<0.001)。三组患者住院期间不良事件发生率呈逐渐减少趋势(20.0%比10.0%比3.3%,P=0.022)。结论随着操作经验的累积,WATCHMAN经皮左心耳封堵术的手术安全性逐渐增加。
